If your pet is scheduled for a spay or neuter procedure, please complete the Spay and Neuter Form before their appointment. This form helps our veterinary team prepare for your pet’s surgery and confirm important details about their health, procedure, and care instructions.

Spay and neuter procedures are common surgeries, but every pet’s needs are unique. Providing accurate information helps us support your pet’s safety, comfort, and recovery.

Complete This Form Before Surgery

Please fill out the form fully and include any important medical history, current medications, recent health changes, or questions you have for our team. If your pet has not yet completed required pre-surgical testing or a pre-surgical exam, please contact us for guidance.

Submit Your Spay and Neuter Form

Please complete and submit the form below before your pet’s scheduled spay or neuter appointment.

The following is included in the surgery cost:

  • Pre-anesthetic exam
  • Pre-surgical pain medication
  • Injectable and gas anesthesia
  • Spay or neuter surgery
  • Post-surgical pain medication

Flea Treatment:

If your pet is found to have fleas, they WILL be treated at your expense.

Prices range from $10 - $20, depending on your pet’s weight.

We make every effort to avoid complications; however, unforeseen conditions may be revealed or may arise during your pet’s procedure that necessitate additional treatments or procedures to be done. Please understand that anesthesia carries risk no matter the age of the pet and that normal pre-anesthetic blood work does not guarantee a smooth anesthetic procedure. Any additional procedures or treatments needed because of anesthetic or surgical complications are the financial responsibility of the owner.
